The gene primarily responsible for extraordinarily high bone density is LRP5 (Low-density lipoprotein receptor-related protein 5). Certain gain-of-function mutations in this gene, most notably the LRP5V171 variant, lead to a rare condition called High Bone Mass (HBM) trait or Worth-type autosomal dominant endosteal hyperostosis. Individuals with these mutations possess bones that are significantly denser and stronger than average—sometimes reported as up to eight times denser—making them nearly immune to fractures from major impacts or accidents. This extreme density is caused by the overactivation of the Wnt signaling pathway, which enhances bone formation by preventing normal inhibitors like sclerostin and Dickkopf-1 from binding to the LRP5 receptor. While this provides unbreakable bones, common side effects include a prominent square jaw (mandible enlargement), bony growths on the palate (torus palatinus), and a lack of natural buoyancy that can make swimming difficult.
